Q16. My name on my application is different from that on my documents (transcripts/awards) Is this acceptable?

If your name now is different to that on your birth certificate or you final award/certificate, you will need to provide official documentation to show your name change. Documents acceptable include: a marriage certificate, a signed affidavit provided you have changed your name by deed poll.
